The Long term disability insurance carrier is UNUN Life Insurance Company.
The LTD insurance program is for Massey employees only. Dependents of employees are not eligible for this program.
The LTD coverage and the cost for the coverage, are determined by the employee’s monthly salary.
Any employee electing not to enroll in the LTD insurance plan must sign the “Declination of Long Term Disability Insurance” statement on the back side of the card.
